Registration

We are happy to register new NHS patients within our practice boundary area. Please see the map below to see if you are within the boundary area.

How to Register:

You can register with the practice using our online form OR by printing the forms on our website and bringing them to the surgery.

Please note, even if you are registering online you must still attend the surgery with documents for the identification check and confirmation. Unless you are aged 15 or under in which case the person with parental responsibility can submit the registration form on their behalf along with bringing the necessary identification documents to the surgery. Please see the list of acceptable documents further down this page.

If registering online, please also print the additional forms (please see below) and bring them to the surgery when you attend for your identification check.

NHS number:

Your NHS number is important to have to hand and to fill in on the registration form. If you do not know it please use this link to find your NHS number: Find your NHS number – NHS (www.nhs.uk)

After you have printed and filled in all forms/registered online, please attend the surgery with the completed forms, one form of photo identification, and one document for proof of address (must be dated within past 3 months)

Acceptable photo identification includes:

  • Passport/Full Driving Licence 
  • Residence Permit Card
  • Rough sleepers’ identity badge

Acceptable proof of address (dated within the last 3 months) includes:

  • Bank statement
  • Council tax bill
  • Utility bill (e.g. gas, electric, satellite television, landline phone bill)
  • Mortgage statement
Identification required for Children:
  • Passport/Birth Certificate (Children only)
  • Red book/record of immunisations (in English)
UNTIL YOU HAVE BEEN SEEN AT THE PRACTICE WITH IDENTIFICATION YOU WILL NOT BE REGISTERED.
