Registering with the surgery

You can register with the practice using our online form or by printing the forms on our website and bringing them to the surgery.

Please note, even if you are registering online you must still attend the surgery with documents for the identification check and confirmation. Unless you are aged 15 or under in which case the person with parental responsibility can submit the registration form on their behalf along with bringing the necessary identification documents to the surgery. Please see the list of acceptable documents further down this page.

If registering online, please also print the additional forms (please see below) and bring them to the surgery when you attend for your identification check.

To register with the surgery:

After you have printed and filled in all forms/registered online, please attend the surgery with the completed forms, one form of photo identification, and one document for proof of address (must be dated within past 3 months)

Acceptable photo identification includes:

  • Passport/Full Driving Licence 
  • Residence Permit Card
  • Rough sleepers’ identity badge

Acceptable proof of address (dated within the last 3 months) includes:

  • Bank statement
  • Council tax bill
  • Utility bill (e.g. gas, electric, satellite television, landline phone bill)
  • Mortgage statement

Identification required for Children:

  • Passport/Birth Certificate (Children only)
  • Red book/record of immunisations (in English)

Important: Please remember

Until you have been seen at the practice with identification you will not be registered.

When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.

Temporary residents

If you are not registered with a doctor but need to see one, you can receive emergency treatment from any GP surgery.

If you need treatment for more than 14 days you will need to register with that GP surgery as a temporary or permanent patient.

Overseas visitors

In April 2015, changes were made to the way the NHS charges overseas visitors for NHS hospital care. These changes also affect some former residents of the UK. The changes were made so that the NHS does not lose out on income from migrants, visitors or former residents of the UK, who may be required to pay for their hospital treatment costs while in England.
